I need a PFD knife and plan on fishing the ocean this coming year. Have looked at a lot online and there are tons of options. NRS Pilot seems popular. Wondering if the regular one will hold up in the ocean of is the titanium version worth the extra $60

Look at the wenoka squeeze. The NRS gets lots of traffic because it's popular and well advertised. The wenoka is a superior quality (in my experience)

I may have to give that a try after my NRS rusts some more, rinsed after every outing and never been used yet but it’s still rusting.

Be sure the knife is designed to effectively and efficiently spread peanut butter and other condiments, because that is probably all you'll ever use it for.
Avoid folding knives, because you probably won't be able to get them open in an emergency situation.
Get a knife with a blunt tip, which reduces the chances that you will stab or cut yourself while flailing around blindly in some emergency situation.
Get a knife which you can sharpen to a very keen edge, and re-sharpen it regularly, because a dull knife won't be very effective in an emergency situation. If you don't have experience in sharpening knives with serrated edges, buy a knife with smooth edges. If you aren't good at sharpening knives with smooth edges, get a friend who is good at sharpening to sharpen the knife for you.
Try to mount the knife on your PFD with its handle up, so if you don't properly seat it in its sheath, it won't slide out and be lost overboard.

I use the non-titanium NRS Pilot knife, and have had no problems. I use it in the salt fairly regularly, rinse it every time, and it hasn't rusted yet. In my opinion, it is not sharp enough to cut fish gills easily, so I usually bring some stainless kitchen shears for that.

If you have a local dive shop, they’ll have choices for a blunt BC (buoyancy compensator) attachable knife that’ll withstand the salt. Wenoka caters to divers.

+1. I have three of these knives. Great knives and keep a good edge. Zip tie the sheath to your vest and you are good to go. After losing one to the ocean, user error, by not having the knife clipped in all the way, I now have a T-Reign zinger attached through the hole in the handle so I won't lose it again.
