I lost the bushing to my rod tip guide on my FethaStyx Chrome salmon rod. Unfortunately, FethaStyx is out of business.  :(
Can I fix this myself? Where can I order the part?

You can buy replacement rod tips and repair adhesive at almost any sporting goods store, this is a common repair.  The tip tops normally come in a 3- or 4-pack that cover a range of rod blank tip diameters.  The adhesive is heat-melt.  Use a lighter to gently heat the busted tip until it comes off (pull/twist very gently with needle nose pliers while you apply heat with the lighter).  To apply the new tip top, heat the end of the adhesive stick to soften it and add a little to the tip of the rod blank (some adhesive from the original tip top will probably be left on the blank).  Then, gently pre-heat the new tip top and the tip of the blank at the same time and slide the new tip top onto the blank.  If you mess up the alignment, you can re-heat it and adjust.  If excess adhesive squeezes out when you slide the new tip top on, you can usually just pinch it off with your fingernails or may need to cut it off with a sharp razor blade.

It's an easier job to perform than it is to describe, and there are probably youtube videos that show the process.  Just search for "rod tip top repair".  I'd find one for you, but I'm at work where youtube is blocked.

Edit:  Something like this is what you're looking for.  http://www.cabelas.com/product/FUJI-HEAVY-DUTY-ROD-TIP/1873443.uts?searchPath=%2Fbrowse.cmd%3FcategoryId%3D734095080%26CQ_search%3Drod%2Btip%2Btop%2Brepair

Too easy. I'll think of a harder question next time. Thanks!

Good response, Low-sky because I just found I have the same problem...again. I'll man-up and do it myself this time instead of farming it out.

Just be careful to not heat up the tip too hot when removing. You can damage the carbon fiber and make it brittle.
Relax. You'll live longer.

Good point. This is why it's important to pull and wiggle while you heat gently when removing. Pulling while you heat ensures the tip comes off as soon as the adhesive loosens up. The amount of heat needed to pull the busted tip top is about how much you need to install the new one. You can also use a candle setting on your desk if you need more hands.

Wow, generous offer, Zach, but you may be a bit too far away for me.
