Once again we are getting event insurance through the American Canoe Association and will be using their waivers.  Feel free to print these, fill them out, and bring them to the ORC.   Please do not email them to me!.

I will have plenty of these printed out at the ORC as well.

A few notes about filling them out...

1.) At the top where it asks about membership, you want the last checkbox... "I would like an ACA Event Membership for $5".  This is the insurance fee that is included with your registration.  If you are already a full ACA member, fill out your ACA number on the form and bring your ACA membership card.   I will refund you your $5 event membership fee that you paid with registration.  If you feel like becoming a full ACA member on the spot, feel free to check one of the other boxes near the top and bring the difference in membership cost to the ORC.

2.) Near the bottom under "Name/Description of the Activity or Event", please enter "Oregon Rockfish Classic"

3.) Near the bottom under "Sponsoring Club / Organization", please enter "Northwest Kayak Anglers"

4.) Under "Activity Date", please enter "July 15, 2017"

5.) Don't forget to fill out the rest of the form including, and most importantly, your signature.
