I think most guys have a knife secured to their PFD. Based on what I have read it sounds like he didn't have a PFD on at the time of the incident. I don't know any hard details but It's a bummer either way...

Yeah it sucks either way, but it looks like a PFD and/or safety knife would have saved the guy.

No confirmation, but seems to add up to no PFD:
